Observation

During onsite inspection, Licensor observed Provider ZA go inside for 2 minutes during visit leaving children with household member MA unsupervised with children outdoors, leave the children in the childcare room with Licensor and go into the kitchen (closing a door behind them) for 13 minutes, leave children in childcare area unsupervised with Household Member MA and Licensor while taking children to wash hands in bathroom, and leave children unsupervised with Licensor while changing children’s diapers. (3) During onsite inspection, while outdoors with children Licensor observed Provider go inside the childcare and out of sight with a child, leaving three children outside unsupervised with Household Member MA and Licensor for 2 minutes.
